FORMS & ADMISSIONS
Welcome to our Forms and Admissions Page. As of the school year 2019-2020, all beginning of year paperwork is now online for all of our schools in SAU 20. Please review the guides under either "New Students or Returning Students" before enrolling/filling out the yearly required forms. If you need any help with filling out the forms, please contact the school.
If you misplaced the letter sent home or need additional assistance please submit a support request by clicking the Support Request button.

Attention parents/guardians of Gorham Middle & High School students. If you have created a Vision(MMS) Parent Gradebook account in the past this is the same account you will use when filling out these required yearly forms. If you have any children that attend another SAU20 School you will need to add them to your existing account by following the Guide for Parents with Existing Vision(MMS) Accounts/Additional Students which will also guide you through filling out your yearly forms. If you already have access to all of your students in SAU20 you can use the Guide for Parents with Existing Vision(MMS) Accounts/All Students.

Parents/guardians of students ONLY in Errol Consolidated School, Edward Fenn Elementary School, or Milan Village School- Letters will be sent home with instructions and some additional information you will need to set up your Vision(MMS) accounts as well as how to fill out the required yearly forms.
